New & Existing Patient Forms
To our extended Dental Family,
We hope that you and your families are faring well. We are beginning to see the light at the end of the tunnel in regards to the current health crisis. During this period of shutdown, we have been working diligently to provide you with the same level of care that you have come to expect from our office, along with some added features to provide you with an increased level of safety in our new normal.
Our patients’ safety has always been of utmost importance to us and now is no exception. In response to COVID-19 we have been attending many continuing education courses, webinars, and reading all of the new guidelines set forth the ADA(American Dental Association), the CDC, PA Department of Health, and OSHA in order to protect both you and our dental team. You will notice not only in the equipment that we use but also in several of the protocols that we now follow. Among our new equipment we have added the following:
The new safety protocols we have established require some changes to the way that we had previously practiced. Although there are changes to our procedures, they do follow regulations established by our government and its agencies and are mandatory in our practice. If you are unable to comply with this, we will be happy to postpone your treatment until these regulations are lifted. As part of minimizing contact with surfaces we are moving to a digital platform to fill out forms such as health histories and office policies. We are asking that all patients provide us with either a cell phone number or an email, in order for us to make your appointment more efficient and remove items that may be touched by others.
The new safety protocol when coming to the office for an appointment is as follows:
As of June 3, 2020 new dental guidelines have been put into place. Per the PA DOH we are now able to provide all dental procedures including elective and routine treatment such as hygiene appointments, restorative, dentures, implants, etc. If you would like to schedule an appointment, please call the office at 570-421-3060.
Thank you for your patience through this time as we provide you with a safe environment. We look forward to seeing you again soon.
